- Address and Patient Phone # – Info for other designated contact for patient
- Any special Instructions and History & Physical
- List of Current Medications and the name of the Primary Care Physician
- Face to Face documentation and Discharge Orders
- Specific Orders required (in advance when possible) for
- DME orders and qualifying diagnosis or O2 with Room Air sats within 48 hours of discharge, Infusion orders and pharmacy , Enteral Feeding orders and supplier
- Diabetic Management orders, frequency, caregivers available for assistance
- Wound Care orders & supplier, Coumadin orders & managing physician
We routinely coordinate DME, O2, Infusion, Enteral feedings, Diabetic and Wound care supplies etc. Advanced notice of a discharge is always helpful and appreciated. But a physician’s order is always required.
