Hospice Care

Providing Comfort Your Way, in Your Home

Hospice Care provides comfort, dignity, quality of life and time to those facing a life-limiting
illness, such as heart disease, Chronic Kidney Disease, COPD, HIV/AIDS, cancer, dementia or Alzheimer’s Disease. In addition to providing compassionate medical care and pain management, hospice care offers emotional and spiritual support to patients and their loved ones.

Over two-thirds of seniors who are eligible for hospice care are missing out on the
opportunity to benefit from comprehensive physical, emotional and spiritual care
and support for themselves and their families – often because they don’t know about,
or don’t understand, the benefits of hospice care.

Hospice care is provided wherever a patient resides, and it is delivered by the hospice care
team- specially trained physicians, nurses, certified nursing assistants, social workers, chaplains and volunteers, who work together to meet the unique needs of each patient and family.
